Submitting health checkup – Migrant workers health insurance documentation

        Foreign workers who wish to submit a health check and health insurance to work in Thailand must prepare personal documents or other evidence for submitting a request for examination with a hospital. The process is as follows:

1. Copy of passport or temporary passport, or copy of travel document or certificate of identity, or copy of ID card. These are for a person without Thai nationality or a copy of the document certifying the registration list (Thor.Ror.38/1) issued by the Department of Provincial Administration;

2. In the case of those who do not have proof of nationality, they must collect data for identification purposes, such as fingerprints or photographs;
3. Copy of national ID card and a copy of the employer's house registration;
4. Copy of ID card and a copy of the reference person's house registration where migrant workers and their dependents live;
5. A copy of the certificate of social security rights or certification documents. Contribution of foreign workers In case the employer is under the law on social security (if any);
6. Work permit application form or work permit, or letter of permission to accept foreigners to work (quota) issued by the Department of Employment (if any);
7. Health check-up fee at the specified rate.

Health insurance documents or evidence documentation

1. Copy of passport or temporary passport, or copy of travel document or certificate of identity, or copy of ID card. This applies to people who do not have Thai nationality issued by the Department of Provincial Administration, or a copy of the document certifying the registration list (Thor.Ror.38/1) or the birth certificate of a person without Thai nationality (Thor.Ror.3). In the case of those who do not have proof of nationality, they must collect data for identification purposes, such as fingerprints or photographs;
2. Receipt or proof of health check-up fee;
3. Receipt or proof of health insurance;
4. Request documents for foreign worker’s health insurance card.
