Documentation for issuance of notification for import-export of Type 2 Hazardous Substance

     Under the 1992 Hazardous Substance Act, hazardous substances include explosives, flammable materials, oxidizers and peroxides, toxins, disease-causing agents, radioactive materials, genetic changers, corrosive materials, irritants, and other substances whether they are chemicals or anything else that may pose a risk to people, animals, plants, property, or the environment.

     These hazardous substances are classified into 4 types based on the necessity of their control. For Type 2, it refers to hazardous substances that the production, import, export, or possession must be reported to the officials beforehand and must comply with the criteria and methods stipulated.

     The issuance of notifications for the import/export of hazardous substance Type 2 requires the following documentation:

1. Notification of operations involving hazardous substances Type 2 (Wor Oor/Wor Kor 1) (1 original copy)
2. Power of Attorney letter (1 original copy) with a copy of the ID card of the assignee, stamp duty as prescribed by law, all copies must be certified true copies, a copy of the ID card of the authorizer
3. Material Safety Data Sheet (MSDS) and all copies must be certified true copies (1 copy)
4. Analysis results, which must only be the Certificate of Analysis of hazardous substances (1 copy)
5. Proforma invoice (1 copy)
6. Document demonstrating the expertise of the specialist for storage, to be used for the first submission (1 original copy)
7. A sketch map showing the storage location of the hazardous substance and the surrounding area (1 original copy)
8. Document showing the characteristics of the container to be used, and the wrapping or bundling of the hazardous substance container (Packing list) (1 copy) and all copies must be certified true copies
9. Consent letter for the use of storage location or lease agreement of the location (1 copy)
10. Permission letter from the owner of the hazardous substance product to use the document for application for export notification of hazardous substance type 2 (1 original copy)
