Supporting Documents for Change of Items in Registration as Maritime Transport Operator

     The maritime transport business, or marine transportation, is considered a form of international trade because it is relevant to the movement of goods from one country to another. However, the transportation requirements or laws of each country are different. To secure and control goods and transportation, government agencies specify the requirements for international transport operators. Those who wish to operate a maritime transport business must apply for a maritime transport license. In the case of an operator having a license but wishing to change any items on the license, the applicant must prepare supporting documents, as follows:

  1. The original application form of change (Form Phor Wor 9);
  2. The original maritime transport operator certificate;
  3. The original documents and evidence issued by government agencies showing the change of items;
  4. The original map showing the location of the maritime transport establishment;
  5. The original company affidavit with establishment objectives;
  6. The original power of attorney (in case of authorization to another person to act on their behalf);
  7. The original identification card;
  8. The original passport (in the case of an alien applicant).
