Supporting Documents for Issuing the Certificate to Export Mollusks and Mollusk Shells Out of the Kingdom

     The operator must prepare the following documents to apply for a certificate to export mollusks and mollusk shells out of the Kingdom:

1. Identification card;
2. Certified (signed) copy of identification card of authorized director;
3. House registration (Thor Ror 14);
4. Certified copy of the company affidavit, issued not more than 6 months before the date of issuing the application document, with establishment objective and company compliance. The original document must be presented on the day of submitting the application;
5. Application form for certificate to export mollusks and mollusk shells out of the Kingdom;
6. Certificate of mollusk species or imported acquisition, identified species of aquatic animal (scientific name), issued by responsible government agencies. In case the officer requires a copy of the certificate, he/she may take a copy from the original;
7. Invoice;
8. Photo of aquatic animals or carcasses or aquatic products to be exported;
9. Power of attorney; a certified copy of the identification card with duty stamp 10 baht/time or 30 baht/year and a certified copy of identification card of the authorized person must be attached.
