Supporting Documents for Applying for Ship’s License of Non-fishing Vessel (First Issue)

The applicant for a ship’s license of non-fishing vessel (first issue) must prepare the documents as follows:

1. Identification card;

2. Alien identification card;

3. Passport;

4. Company affidavit, issued not more than 6 months before the date applying for the ship’s license, certified by the company seal stamp;

5. Identification card of authorized director;

6. Identification card of the head of the government agency;

7. Identification card of authorized person;

8. Application form (Form Kor 5);

9. Certificate of survey (Form Tor Ror 20-1 Kor);

10. Proof of hull and engine acquisition according to Form Kor. 

11. Proof of hull and engine acquisition according to Form Khor. 

12. A letter from a government agency or state enterprise appointing or authorizing anyone to act on its behalf;

13. Power of attorney with stamp duty, downloaded from the website:

http://oldweb.md.go.th/srb/source/form.php; 

14. Proof of vessel ownership; 

15. Full-side photos of the vessel: 

    • One port-side photo and one starboard-side photo;
    • A vessel inspector records the number of the certificate of survey and signs on the back of photos.

16. Mutual of agreement between buyer and seller;

17. Certificate of deletion from the responsible officer of such country;

18. Proof of gift or inheritance in lieu of proof of acquisition of hull and engine;

19. Insurance contract and passenger insurance policy.
