Supplementary Documents for the Counseling Process Prior to Relinquishing Custody of an Adopted Child Who Is Still a Minor

     The Ministry of Social Development and Human Security has established the following documentation as a requirement for the counseling process for the preparations prior to relinquishing the custody of a minor child:

  1. National ID card;
  2. House Registration Certificate (Thor Ror 14);
  3. State Officer Identification Card;
  4. Government Organization Employee Identification Card;
  5. Birth Certificate (Thor Ror 4) (all certified true copies of the original documents are required to verify the child's true parents);
  6. Death Certificate (Thor Ror 4);
  7. Court Order or Final Judgment of the court;
  8. Adopted Child Certificate Registration (Kor Ror 14);
  9. The document indicating the change of one's first or second name (Chor 3) to reflect one's identity, since some individuals may change their names subsequent to the issuance of their the Adopted Child Certificate Registration;
  10. Marriage Certificate (Kor Ror 2) indicating parental authority over the child; 
  11. Divorce Certificate indicating and verifying evidence of parental authority over the child. 

     All certified true copies of documents are required.
