Issuing a medical certificate – Reporting results of foreign workers’ health check

       When a foreign worker is given a mandatory health check-up, there is a process for issuing a medical certificate. It must be a medical certificate in the prescribed form.

       Various items of information should be filled in, such as the out-patient medical record number of the hospital (OPD Card), examination location information, the name of the responsible hospital, the name of the foreign worker, or dependents, age, address, use the name from the copy of the passport (Passport), or a copy of the temporary passport (Temporary Passport), or a copy of a travel document (Travel Document), or a copy of a document certifying a person (Certificate of Identity), or a copy of an identity card for a person who does not have Thai nationality issued by the Department of Provincial Administration, or a copy of the document certifying the history of registration (Thor.Ror.38/1), such as Myawaddy, Koh Song, Dawei, Malayaeng (for epidemiological results and routes into the city).

       Information on nationality and ethnicity should be clearly stated. Data entry may be handwritten or typed, and signed by the examiner with the seal of the hospital. The doctor must summarize the results of the examination, as follows:

  • Good health refers to the result of Category 1 examination;
  • Passed a health check, but requires treatment or follow-up means the results of the examination are Category 2;
  • Failed health check means the results of the examination are Category 3.

       The medical certificate is valid for 60 days from the date of health check-up, except for health insurance, which is valid for 1 year from the date of health examination.

Health check-up results report is divided into two cases.

In case of "Pass"

         The medical facility provides a medical certificate in a sealed envelope and signs it, adding 1 stamp of the medical facility. The facility should send a copy of the results directly to the Provincial Public Health Office in the case of provincial areas for hospitals in Bangkok, and send a copy of the results to the Medical Department. The certificate can be used to apply for a work permit.

 In case of “Fail"

        The health check-up facility must put a red rubber stamp on the medical certificate, with the statement “Return” and proceed as follows:

  • Hospitals located in regional areas: A copy of the health check result must be sent to the Immigration Office so that they can proceed with repatriation. It should be sent to provincial employment agencies so that they can cancel the work permit;
  • The hospital coordinates the treatment of the patient until the end of the contagious period to expedite repatriation to the country of origin as soon as possible. During the treatment, the patient and the employer are responsible for the expenses incurred.
