The Minister of Public Health has issued a ministerial regulation on fees and payment of fees for the operation of a health establishment B.E. 2560 (2017) of the Health Establishment Act B.E. 2559 (2016) as follows.
Clause 1. Fees shall be charged
(1) For a health establishment license for spa business
(a) 1,000 baht for a health establishment with service area not exceeding one hundred square meters
(b) 3,000 baht for a health establishment with service area exceeding one hundred square meters but not exceeding two hundred square meters
(c) 6,000 baht for a health establishment with service area exceeding two hundred square meters but not exceeding four hundred square meters
(d) 10,000 baht for a health establishment with service area exceeding four hundred square meters
(2) For A health establishment license for massage business for health or beauty purposes.
(a) 500 baht for a health establishment with service area not exceeding one hundred square meters
(b) 1,500 baht for a health establishment with service area exceeding one hundred square meters but not exceeding two hundred square meters
(c) 3,000 baht for a health establishment with service area exceeding two hundred square meters but not exceeding four hundred square meters
(d) 5,000 baht for a health establishment with service area exceeding four hundred square meters
(3) 1,000 Baht for a health establishment license for practitioner
(4) the fee equal to the license fee under (1) or (2) for each renewal of a health establishment license
(5) 300 baht for a substitute license under (1), (2) or (3)
(6) 300 baht for each amendment to the particulars in the license under (1), (2) or (3)
(7) 1,000 baht annual fee for a health establishment license under (1)
(8) 500 baht annual fee for a health establishment license under (2)
Clause 2. The service area of a health establishment under Clause 1 shall be calculated according to the floor plan drawing showing the service area of a health establishment submitted together with the application for a license or license renewal, as the case may be.
Clause 3. An applicant for a health establishment license shall pay the annual health establishment fee for the first year and the license fee. It shall be deemed that the payment date of such fees is the due date for payment of the annual health establishment fee in subsequent years throughout the period of its operation. When the licensor has received the correct and complete payment of the fee under paragraph (1), the licensor shall issue evidence of the fee payment to the licensee in accordance with the form prescribed by the Director-General within three days from the date of receipt of such fee payment.
Clause 4. The evidence of payment of the annual health establishment fee shall be displayed in a conspicuous and visible place at the health establishment.
Clause 5. An application for fee payment in accordance with this Ministerial Regulation shall be submitted at the following places.
(1) In Bangkok, at the Department of Health Service Support, Ministry of Public Health
(2) In other provinces, at the Provincial Public Health Office
(3) Other places as prescribed by the Director-General in the Government Gazette, taking into account the convenience and burden reduction for the applicant in filing an application under paragraph (1), the applicant may submit an application via electronic system in accordance with the rules, procedures and conditions prescribed by the Director-General. The application for fee payment under paragraph (1) shall be in accordance with the form prescribed by the Director-General in the Government Gazette.
