Application to stay in Thailand in the case of medical treatment, rehabilitation, or patient care

      An application to stay in Thailand in the case of medical treatment, rehabilitation, or patient care has the following requirements: 

      In the case of medical treatment or rehabilitation or patient care:

1. Foreigners must be certified and requested by the attending physician and present details about their illness, duration of treatment, and the attending physician’s opinion stating that the illness is an obstacle to travel;

2. In case of patient care, foreigners must be certified and requested by the attending physician or the embassy or consulate;

3. Not more than one caregiver, other than parents, spouse, child, adopted child, or spouse's child, is permitted. 

      Foreigners who enter the Kingdom to receive medical treatment and their dependents, and who are exempt from visas and are permitted to stay in the Kingdom for no more than 90 days, shall be nationals of the Kingdom of Bahrain, Kuwait, Sultanate of Oman, Qatar, Saudi Arabia, United Arab Emirates, and other countries as announced by the Ministry of Interior:

1. Patients and caregivers must be certified and requested by a medical facility in Thailand, according to the list prescribed and issued by the Ministry of Public Health;
2. The patient’s caregivers must be his/her parents, spouse, children, adopted children, or other persons, according to the document certified by the patient, and not more than 4 persons.

Required documents

1. TM. 7 form;

 2. Copy of the applicant's passport;

 3. Confirmation letter by the attending physician; 

 4. Only in the case of patient care, a letter from the attending physician and proof of relationship (in the case of a caregiver who is part of the household), such as proof of marriage, a copy of the birth certificate, or registration of legitimation of child (certified by the embassy or consulate), must be presented. 

Required documents in the case of foreigners and their dependents entering the Kingdom to receive medical treatment,, who are exempt from visas and may stay in the kingdom for not more than 90 days, for nationals of Bahrain, Kuwait, Sultanate of Oman, Qatar, Saudi Arabia, United Arab Emirates, and other countries as announced by the Ministry of Interior:

1. TM.7 form;

 2. Copy of the applicant's passport;

 3. Confirmation letter from a medical facility in Thailand, according to the list prescribed by the Ministry of Public Health or from the Ministry of Public Health; 

 4. Patients must attach documents related to medical treatment issued by a medical facility in Thailand according to the list prescribed and examined by the Ministry of Public Health, including medical appointment documents, medical treatment certification documents from physicians, and medical treatment confirmation documents, or other types of medical documents;
5. Caregivers who are the patient's father, mother, spouse, child, or adopted child must present proof of family relationships, such as a copy of the house registration, a copy of ID card, a copy of marriage certificate, a copy of the birth certificate, a copy of registration of legitimation of child, or a relationship certificate from the patient in case the marriage is not registered. For caretakers who are not the patient's parents, spouses, children, or adopted children, an employment contract or certification letter from the patient must be presented. 

      In both cases, a document showing the names of the patient's caregivers must be attached and certified with the patient’s signature.

      The aforementioned documents must be certified by an embassy or government agency of a member country in the Gulf Cooperation Council according to the form of the certificate of relatives and dependents of the medical service recipient, translated into Thai or English.
