In the case of adoptive families from foreign countries who accept orphaned children under the care of the Department of Children and Youth, the following documentation and evidence must be provided when requesting to adopt a child:  

1. National ID card;    
2. House Registration Certificate (Thor Ror 14);  
3. State Officer Identification Card;  
4. Government Organization Employee Identification Card;  
5. Passport;  
6. Birth Certificate (Thor Ror 1);  
7. Juristic Person Certificate; 
8. Document indicating the change of one's first or second name (Chor 3) to reflect one's identity;
9. Marriage Certificate (Kor Ror 2);  
10. Divorce Certificate   (Kor Ror 6); 
11. Work Permit (WP.11);  
12. Request Form for Adopting a Child (Bor Thor 5);  
13. Results of Criminal History Check (showing the history of criminal offenses or child abuse);
14. Preparation for the acceptance of foster  families;
15. Divorce Certificate (Kor Ror 6) (the right to exercise parental authority over the child);  
16. Four 2-inch photos (of the adopter, spouse, and any related parties);  
17. Photos of the inside and outside of the residence;
18. Documents from the relevant foreign agencies (confirming that the applicant is qualified to adopt a child);
19. Certification Letter (from the immigration authorities or relevant agencies of the country where the applicant for an adopted child is from, certifying that the applicant has a permanent residence and can legally bring the child to the country in accordance with its laws);  
20. Letter of recommendation from acquaintances (at least 2 persons);  
21. Home Study Report;    
22. Certificate of endorsement from the organization responsible for monitoring the child fostering program (certificate of endorsement from the organization that monitors the child fostering program every two months from the relevant organization);  
23. Confirmation letter from an authorized agency (stating that the agency will undertake legal procedures within the country once the process of adopting a Thai child is complete);
24. Applicant's personal history;
25. Medical certificate of physical health;
26. Medical certificate of mental health and the results of psychological testing;
27. Letter certifying employment and income;
28. Letter certifying financial status for the past 6 months;
29. Letter certifying assets.

     All copies of the documents provided must be certified as true copies.
