Introducing the "THALANG MODEL" – A Seamless, Comprehensive Healthcare Network for the Elderly

Thalang Hospital in Phuket has initiated the “THALANG MODEL,” a comprehensive healthcare program for the elderly, emphasizing seamless care from home to hospital. The project is based on the concept of team collaboration (T-Team Work) among the multidisciplinary team at Thalang Hospital, local health centers (RPH), and related networks. The primary goal is to enable elderly individuals to live normal, active lives, maintain their abilities, and contribute to their community and society.


One of the key features of the THALANG MODEL is health screenings in nine areas for the elderly: vision, hearing, mobility, oral health, urinary incontinence, cognitive memory, daily activities, malnutrition, and depression. This screening is conducted by local health volunteers and public health workers. If any risks are identified, the elderly receive appropriate care and referrals. Additionally, the model establishes a long-term care system for dependent seniors, covering all sub-districts in the Thalang area, with individual care plans to ensure continuous and organized care.


Another significant activity under the THALANG MODEL is the establishment of elderly clubs and schools in every sub-district, as well as intensive behavior modification camps to enhance the health literacy of the elderly. The focus is on self-care to prevent diseases and various risks. The ultimate goal of the THALANG MODEL is to ensure that the elderly can live quality lives, stay healthy, and fully participate in their communities.
