Submitting Health Checkup - Migrant Workers Health Insurance Documentation.

        Foreign workers who wish to submit a health check and health insurance to work in Thailand must be prepared personal documents, or other evidence for submitting a request for hospital examination as follows:

Documents or evidence for health check documentation.

1. Copy of Passport or Temporary Passport or copy of Travel Document or Certificate of Identity or copy of a person without Thai nationality’s ID card or a copy of personal history registration certifying list (Thor Ror. 38/1) issued by the Department of Provincial Administration.

In the case of those who do not have proof of nationality shall prepare data for identification purposes, such as fingerprints or photographs, etc.

2. A copy of the identity card and a copy of the employer's house registration
3. Copy of ID card and a copy of the house registration of the reference person where the migrant worker and his dependents reside.

4. A copy of the certificate of social security rights or documents confirming the contribution of foreign workers in case the employer is under the law on social security (if any).
5. Application for a work permit or a work permit or a letter permitting foreign worker to work (quota) issued by the Department of Employment (if any).

6. Health check-up fee at the specified rate.

Health insurance Documents or evidence documentation.

1. Copy of Passport or Temporary Passport or copy of Travel Document or Certificate of Identity or copy of a person without Thai nationality’s ID card or a copy of personal history registration certifying list (Thor Ror 38/1), or a birth certificate of a person who does not have Thai nationality (Thor Ror 3)

In the case of those who do not have proof of nationality shall prepare data for identification purposes, such as fingerprints or photographs, etc.

2. Receipt or proof of health check-up fee.
3. Receipt or evidence of health insurance fee.
4. Documents for foreign workers health insurance card applying.
