Documents Required for Changing Maritime Zone Application

     Applications for changing maritime zones can be submitted to the Ship Registration Standards Office of the Marine Department or at a regional Marine Office branch. The necessary accompanying documents are as follows:

  1. National Identification Card
  2. Alien Registration Card
  3. Passport
  4. Certificate of Incorporation (each copied document must be certified as a true copy and affixed with the company seal)
  5. Copy of National Identification Card of the authorized signatory in the case of a legal entity
  6. Copy of Alien Registration Card of the authorized signatory in the case of a legal entity
  7. Copy of Passport for foreign nationals who are the authorized signatory in the case of a legal entity
  8. Copy of National Identification Card of the head of the government agency or state enterprise
  9. Application form (Form Kor 5)
  10. Ship's Particulars License
  11. Certificate of Ship Inspection (Form Tor Ror 20-1Kor)
  12. Authorization letter from the government agency or state enterprise assigning someone to act on their behalf, along with the assignee's National Identification Card
  13. Power of Attorney letter, along with the attorney's National Identification Card and the identity documents of the grantor. These can be downloaded from the website: http://oldweb.md.go.th/srb/source/form.php
