According to the Department of Fisheries, Ministry of Agriculture and Cooperatives, the documents required for issuing the aquatic animal health certificate for export are as follows:
- Identification card;
- Certificate of affidavit, issued within 6 months from the date of application. The applicant shall bring the original certificate of affidavit on the day of application. All copied documents shall be certified (by signing), together with the objectives of the juristic person, the articles of association, list of managing partner names/ commercial registration certificate (if any)/ Certificate of VAT Registration (Por Por 20) (if any);
- Identification card of the authorized director of the company. All copied documents shall be certified true copies by signing them;
- The application for aquatic animal health certificate (Kor Por Mor, Sor Sor 1);
- The registration certificate of aquaculture establishments for export (Sor Or 3);
- The registration certificate of aquaculture collection establishments for export (Sor Or 4);
- Aquatic Animal Fry Movement document (Jor Sor Nor 2);
- Aquatic Animal Movement document (Jor Sor Nor 3);
- Aquatic animal invoice;
- The aquatic disease test report from aquatic disease examination laboratories, as in Annex 2, or as announced and determined by the Department of Fisheries. The aquatic disease test report shall be valid for no more than 30 days from the date of collecting samples;
- Power of attorney with a copy of the identification card of the authorized person and stamp tax as prescribed by law. All copied documents must be certified by signing;
- Documents that the importer sets additional conditions for importing aquatic animals, e.g., fish resting records and the updated aquatic animal certificate of the destination country, which are not available in the system.
